Patient Referral
It is usual for a patient to be referred to us via their dentist. However if you are a private patient you can self-refer, there is no need to be referred via your general dentist.  Please use the patient referral form on this website or contact us by telephone or email.

NHS patients are always referred to us by their general dentist.

General Dental Practitioner Referral
As a GDP you can choose to refer to us by letter, phone call, fax or by completing the referral form contained within this website. If you elect to use the web from please include as much information as possible about your referral. The referral form will be received by our Practice Manager who will respond in the usual way.

Smiles Orthodontics Practice can offer expert treatment and advice to adults and children. The services range from simple interceptive procedures through to multidisciplinary care and orthognathic treatment. You can refer any patient with either a developing or full malocclusion for advice and treatment.
